Dr. Hanaway's services are currently limited to direct physician referral.  Care providers should submit a written request, health history summary and patient consent to Family to Family for review.  Upon acceptance of the case, an initial face-to-face intake with the patient is required.  All other follow-up can be managed via phone or encrypted email.
